A completed application for the Initial Workshop Leadership Training includes the form available for download below, completed, and a $300 non-refundable deposit towards the training tuition.

Please fill out the above form and send it, along with contact information for two references and your writing sample, as an attachment via email to email@example.com or via postal mail to the address below.
Once accepted, the $300 is a non-refundable deposit towards the $2250 training tuition. If your application is not accepted, $250 of the deposit will be returned less a $50 administrative fee.
